Enrollment Process for New Adults (age 18 and older)
Adult volunteers may enroll starting July 1st and any time during the program year.
Each prospective volunteer must complete the four phases of the enrollment process before they will be allowed to work with 4-H youth. Once you have completed the phases and are confirmed in the 4hOnline enrollment system we will then mail you your 4-H Leader Card.
Prospective Volunteers may not lead projects or represent the 4-H Youth Development Program in any manner until they receive their 4-H Leader Card.
Phases 1, 2, and 3 require Internet access. If you do not have Internet access, please Julie Katawicz, 4-H Program Representative to schedule a time to use the computer and Internet in the 4-H Office.
Phase 1: Enroll online and pay $17.00 insurance fees.
All new volunteers must enroll themselves in 4hOnline starting July 1st.
Give this form along with a check/cash for insurance fees to the 4-H Club Enrollment Coordinator. Make checks payable to the 4-H Club.
The Club’s Enrollment Coordinator is to serve as a guide in helping individual families in accessing 4hOnline and completing their enrollment. If a family does not have Internet access, they may turn in an paper 2017-18 Adult Volunteer Application Form to the Enrollment Coordinator, who will enter it into 4hOnline on their behalf.
Phase 2: Fingerprinting
Complete and Print the following:
4-H leaders are responsible for paying the Live Scan fee shown on the location sheet (it varies with each location). However, you can submit a check request to get reimbursed for the Live Scan Fee from the San Benito County 4-H Council.
You must have a copy of the Live Scan Applicant Submission Form with our information on it in order to have your fingerprinting done. After completing your fingerprinting, please keep a copy of the submission form for your records and give a copy to your Club Leader or Enrollment Coordinator along with the receipt and your check request for reimbursement.
